It wouldn't actually be the first time either. It's somewhat of an in-joke with some artist. The artist of that strip is Esuyuki, who does a lot of Asakyon - he's the one that made the series where she makes child using their DNA and data manipulation - this strip is from Ra~men, who often does Tsuruya x Kyon (Tsuruya's "special place" is her forehead...)

I've seen the bit done with Tsuruya several times before, though I'm still relatively surprised this is the first time I've seen the gag done to Ryoko.

Oh, and all in all, I'd have to say that the choice between Ryoko, Tsuruya, (and apparently Mikuru) is a really tough choice. Too bad he pissed some people off and now he got his ass kicked.

Well, you saw Mikuru at the end of the doushin, didn't you? So to Kyon, his thoughts of his dream girl had a third choice... and apparently, due to the less-than-happy reactions of the other two girls, he made the wrong one... Hence, he got his ass kicked.

It isn't wise to mess with a smoked-cheese addict and a cute, knife-wielding psychopath... If Kyon valued his life properly, he would have chosen one or the other (although... I wouldn't exactly blame Kyon for voicing his truthful opinions...

)
