Must've been the psuedo-chess battling. They tried to demonstrate that they're both very strategic minded in the vein of the Cornelia/Zero battle from Season 1, but it did drag a bit long. That doesn't mean I want Mobile Suit Bullshit fights back, though. This is war, treat it like one, IMO.

I don't think we saw her die, but my stream was rather choppy, and given the "Blink and you'll miss it" feel of this episode, she might have.

I have nothing against the Chess style, but the episode was just.. boring. Nothing really happened.

I don't believe she has the gonads or arguably the heart to kill Lelouch. This is just a setup for the climax.
