Plenty of stuff, and by that, the location of damage can be varies. Coughing blood is coughing blood though, so I assume it's either throat or gaster, damage in that section can cause heavy internal bleeding.

I don't know, never finding such case myself

In case of rib piercing the liver though, you'll definitely need one. I don't know precisely how but this also cause blood form the mouth somehow.

@ Senpai - It just seems like people get owned and cough blood in anime then they shrug it off and are fine by the next scene.
