I don't normally put attention in music when I watch anime or play game, but one I really like is Shōji Meguro.

I think I remember the music from anime I like more than the composer names. ;( But here they are after looking them up. ;o

Toshihiko Sahashi - I like his piano and strings in Gunslinger Girl. In Full Metal Panic TSR, he sets the mood and builds up tension well in some scenes, and I liked how he uses classical instruments mostly for it.

Yasunori Mitsuda - I love his music in Chrono Cross

Yuki Kajiura - I guess I liked her music since Mai HiME, and then there's Kara no Kyoukai as well.

Taku Iwasaki - I've liked his caughty music in Read or Die OAV, and the opening and closing pieces in Kenshin Reminiscence.

* Last EXILE, most of it in general
* Karas, the opening theme comes to mind
* Trinity Blood, I liked the music used in some of the scenes

Forgot to add him to my list of favorites , But I will say some songs sound excellent or terrible with or without gameplay. It depends really...

Well, when it comes to mainstream games, I consider the holy trinity Nobuo Uematsu - Jesper Kyd - Jeremy Soule.

Some other game OSTs that I ranked highly are by Jack Wall, Alexander Brandon and Kunihiko Ryo.

Yuki Kajiura

Only one I know because I liked her soundtracks so much. I normally don't pay any attention to it.

Concerning anime:

Yoshihiro Ike - I absolutely loved the music he composed for Ergo Proxy.
Yuki Kajiura - Her music for Kara No Kyoukai won me over.

Concerning games:

Masashi Haumazu - Severely underrated.
The JDK band - Listen to the music of any game made by Falcom, preferably the Ys series, and you'll understand why.
Akira Yamaoka - The Silent Hill series wouldn't be what it is without hi input.

Plus one that concerns both fields:

Hitoshi Sakimoto - How this guy has managed to consistently compose so many soundtracks of a very high standard over the years, i'll never know. Hats off to him.

You might want to see this thread.

Mine for anime are pretty much the same as I posted there aside for Yuki Kajiura taking No.1 spot after Madoka and F/Z and Kousaki Satoru moving to second with more recent series to help my opinion on him(putting Maeda Jun at 3rd now).

For games only composers the ones who impressed me enough to remember would be Chikayo Fukuda(Did the great OST for the original .hack game series as well as //GU's sound track), Kota Hoshino's work for the Armored Core series, and others would mostly be some of the Final Fanasty composers, well specifically the team who the XI's OST Nobuo Uematsu, Naoshi Mizuta, and Kumi Tanioka(bias since XI still has my favorite game OST to date , having such a massive tracklist helps it ).

This, so much, as well as Namco Sound Team's Ace Combat branch. I can't think of any other videogame music composers that come even close.

When it comes to anime soundtracks, Sano Hiroaki (Nanoha), Toshihiko Sahashi (FMP, Zipang) and Hideki Taniuchi (Death Note, Kaiji) are definitely up there. Kajiura Yuki too, although the only series I've watched that she worked on is Madoka. Iuchi Maiko (Index, Railgun) and the KEY sound team make good stuff too.

I can't believe no one mentioned Takanashi Yasuharu, you know, the guy who did Naruto Shippuden & Fairy Tail's soundtracks?!

He's my no.1 top composer for Anime music!

Wasn't really impressed with Yuki Kajiura's work UNTIL i listened to the recently released Fate/Zero Vol.1 sound track and damn was i amazed.

Kawai Kenji also has some pretty decent work with Fate/Stay Night but Jun Maeda's work with Clannad is second to none other other than Takanashi Yasuharu's work.

Anime

Jun Maeda 麻枝 准 (haruhi, lucky star, ore no imouto, a-channel, angel beats!)
Satoru Kōsaki 神前 暁 Shinji Orito 折戸 伸治 Magome Togoshi 戸越まごめ (clannad, kanon, air)
Hajime Hyakkoku 百石 元 (K-On! original soundtrack)
Takeshi Abo 阿保 剛 (Steins;Gate)
Takeshi Watanabe 渡辺 剛 (Saki TV)
Sumio Shiratori 白鳥澄夫 (Moomin 1990 TV-series)

Games

Frank Klepacki (kyrandia, lands of lore, c&c series like tiberian dawn, red alert 1 and 2, tiberian sun)
Yasunori Mitsuda 光田 康典 Nobuo Uematsu 植松 伸夫 (chrono trigger and cross, final fantasy 6 and 7)
Motoi Sakuraba 桜庭 統 Shinji Tamura 田村信二 (Tales of Phantasia, Star Ocean)
Yasunori Shiono 塩生康範 (Lufia I and II)
Hiroki Kikuta 菊田 裕樹 (Secret of Mana 1 and 2)
Izuho Takeuchi 竹内出穂 Masaki Nakagaki 中垣斉己 (Phantasy Star III and IV)
Masato Nakamura 中村 正人 (Sonic 1 and 2)
Jun Ishikawa 石川 淳 (Kirby series)

Some of these animes and games have so many different composers so I was too lazy to search all of them. I have no idea, if kanji or romaji are 100% correct. Copy pasted them from wikipedia, anime network, etc.

For video games:

Ishiwatari Daisuke- though his music is kind of distracting in the sense that I feel the urge to head bang or air guitar once the song starts.

ZUN- Touhou's music is ridiculously catchy and that's probably why his songs have been remixed countless times. I was kind of surprised that when I went back to listen to the very first Touhou game's OST that I ended up liking it because I assumed that it wouldn't be as good due to inexperience.

Others (in no particular order):
Sugiyama Koichi
Mitsuda Yasunori
Kondo Koji
Yokota Mahito
Uematsu Nobuo
Jesper Kyd

Anime (in no particular order):
Sawano Hiroyuki
Iwasaki Taku
Kanno Yoko
Kajiura Yuki
Sahashi Toshihiko
Tanaka Kohei

Wait what? I'm certain that Maeda never took part of any of these series, except Angel Beats!.
Where did you see such mentions?

My personal favorite is definitely Yoko Kanno

But what is so great about her pieces of music? It is the feeling when the song start playing and I suddenly feel my body jamming and there is nothing I can do to stop the movement. Nowadays I have to wear seat belt when I listen to music composed by her because I always fall in a strange and probably would get injury from a hardcore jamming session.

Hiroyuki Sawano - of Gundam Unicorn and Guilty Crown fame. He's a relatively new composer whose music has fast become one of my favourites.
Yoko Kanno - her music in RahXephon, GitS and Macross F remain my absolute favourites.
Kenji Kawai - him, Oshii Mamoru and Production I.G. are the holy trinity.

...and I'd throw in Hans Zimmer for his tracks in Blood+ just for the heck of it. :P

Yasunori Mitsuda - since Chrono Cross and Xenogears...and have you heard kiRite? Amazing album.
Masashi Hamauzu - Always liked the stuff he did for the SaGa games, but his recent work from Final Fantasy XIII has amazed me
Shoji Meguro - Something different from your typical VGM composer. Really catchy stuff
Yuki Kajiura - Fell in love with her work since Noir, and the music she did for Xenosaga II and III? Awesome.
JDK Band - The arranged versions of a lot of Falcom tunes...so good. And it matches so well with the games, especially the Ys games with all the fast-paced action-RPG fun to be had.
Michiko Naruke - The Wild Arms series has always been one of my favorite game series, so I guess this is more of a nostalgia factor. She gives nods towards Ennio Morricone's work from classic spaghetti westerns.
