Ryougi Shiki (says it at the top of the page actually) from Kara no Kyoukai. It's not an anime though, she's from a novel (and you can find translations here)

It looks like we may be in for a lil' treat here, the official F/SN anime website has posted up information on a Out Tracks album for the OST - meaning we could just be getting those few missing tracks, and the two special ED songs.
