The closest thing I've seen in an anime to Fallout is The Third: The Girl With the Blue Eye. It has a lot of parallels to the post-apocalypse setting used by Fallout, but notably lacks FO's emphasis on humor and retrofuturism. It instead adds some quasi-mysticism for its added flavor.

Now and then, here and there

Fist of the North Star

GunxSword

These are only animes that i know set in a post apocalyptic world.

The problem is Fallout is fairly unique and uniquely American (hooray for 1950s-esque jingoism and McCarthyism!). I don't think a Japanese developer could even really approach the correct tone for making something similar to Fallout.

These suggestions are more generic post-apocalypse settings. None of them have Fallout's quirks.

Desert Punk might fit for you. It has a good deal of comedy, action, the main character goes around doing bounties which could be seen as missions. Overall it's pretty funny and pretty good. Oh and it is set in a post-apocalyptic world.

I'm not sure if you like mecha series and stuff but Gundam X is kinda one of those "hey the world get screwed up and oh snap we're still alive somehow".

Wouldn't the M.D. Geist animes count on what you are asking for?

Though not an anime, I thought planetarian was very fallout-ish in it's presentation.

Big O would have been much better if it weren't for the Instant Awesome Just Add Mecha bit. It had a good Gotham City-esque, Batman: TAS vibe going on... then huge senseless robots show up.

Overman King Gainer is one of the best shows i've ever seen, so i'd recommend it just for that . But yeah it does have a post-apocalypse setting.

You should know that they censored the nuke scene from fallout 3 in the japanese version, With the thinking that Japanese are sensitive to that kind of imagery.

So it might be difficult to find an anime with a fallout like presentation that isn't about giant robots roaming the land.

Even ignoring censoring... as others have said Fallout is such a unique-ish thing that I doubt you're going to find something closer than a post Apocalypse style anime, which are common but not the same as fallout.
