I would assume:

It was the newly reverted Nagato who visited Kyon at the hospital (he even said so in vol.7) and went through Lover; Snow Mountain Syndrome; Where did the cat go?; etc and all the events up to January 2nd, when she went back with Kyon and Mikuru-chan. Only then did future Nagato come out in the open again, AFTER the three have gone. A few minutes later, only Kyon and Mikuru-chan come back. This "future" Nagato who just stepped in IS the one that just disappeared (just with an extra 2 weeks on top of her age), therefore she replaces her, gets back on track and and carries on her life with the gang. Its still the 'same' Kyon and Mikuru-chan she's reuniting with, she just took the long way to get back, whereas them two time-travelled to come back. The Nagato that just disappeared will basically go through all that 'hiding behind the scenes' phase as well and eventually "become" the future Nagato who just stepped in, thus creating a loop.
This just means that she'll age an extra 2 weeks. Thats only a little more than Mikuru-chan aging an extra one week in vol.7, and its NOTHING compared to what she went through in Endless Eight.
EDIT: I just read that part of vol.7 again and I realised it CAN be interpreted that Nagato did just go back with them. If this is the case, then it means that its the newly reverted Nagato who applied the changes like 'Kyon fell down the stairs' and everything else. But the theory that she stayed behind seems to make more sense for me.

Thats true. Besides the car chase, I don't think there is much to add in the cinematics/effects sense. But then this is KyoAni, they're pretty good at bringing out surprises when you don't expect it.
