I already explained that earlier as well. Yuuki felt that Chisato was using him as a replacement for her little brother and that is keeping him from further advancing his relationship with Chisato (plus i think the promise that he made as a child to protect her had something to do with it as well )

I am pretty sure it was car accident . It would have been a little bizarre if it were by chocolate. This is not like Chaos;Head when someone died from drinking coke (eventhough it was a delusion)
