My theory:

Eureka returns in ep14, and she gives birth to Elena, who she brings up in that world, but the environment might not be good for her. Years later, she is pregnant again, this time with Ao, and she returns to the world to give birth to him. Somehow, between these two events, Elena is abandoned in Ao's world as well. Either because she tried to follow Eureka, or some accident happened.

Also it doesn't click to Eureka why Ao calls her Mama cause she is pregnant with a girl now. It likely dawns on Eureka who Ao is upon her second pregnancy.

Whatever circumstances that relates to Elena led to Eureka coming back for Ao's sake.

Elena probably hates Eureka because from her POV she abandoned her. Elena empathizes with Ao since they have the same raw deal. Plus Trapar Miller speculating Elena is related to it makes some damn sense if Elena is half Coralian.

Though her medical data is likely kept under wraps by the president. Also the surprise of the Americans on the DNA results. Protecting Elena is why the president wanted Gazelle to drop his inquiry.
