Index just develops very slowly, you know...
It wasn't till volume 15 that 2nd and 4th were introduced.
I wouldn't worry very much about that. He/she will appear in time.

Btw, for people who read the novels.
I was told that in volume one, Mental Out was hinted.
Specifically, Touma called Komoe to ask for a "certain telepath" to take care of Index's problem.
Can anyone confirm this?
