Maybe Ichigo is really Shiba Kaien (since everyone knows they look so much alike) and Urahara resurrected him after Rukia killed him but he lost his memories... and since the Hollow took over his body and their souls merged so he always had the hollow in him but it only came out in the pit. and um.. all the memories of his childhood were planted/faked.

it's imposible to plant Ichigo's memories as there is too many people that share them with him...
I don't think that Ichigo is reincarnation of Kaien after anime didn't make all those scenes that would bring that thought
