That's something I don't understand as well. If you ask me, it's probably time. The things with Tatsuya's Divine Left is that it allows him to restore and regenerate an object, human, or injury the way it is at any time within a 24 hour time limit. In all likelihood, he received a lot of his scars from before he had a proper grasp of regeneration and as such is outside the 24 hour rule. After all, he was recognized as a defect early on and experimented/surgically operated on at the age of 6. He most likely did not really understand his own abilities yet.
Also, as great as divine left is (and maybe someone can better explain this), Miyuki heavily disapproves of him using it to save people. When the other council members thought Tatsuya's healing magic was temporary (because most are), Miyuki rebutted by stating that the heal is permanent. After some explanation, the other members got excited, but Miyuki still had a very dark face because in order to use it, Tatsuya had to access the 24 hour information on the object and human within a moment of time and then rewrite/regenerate the information to the way it was before. In other words, a soldier's pain from combat are all sent to Tatsuya's mind within 2 seconds. It's like a super-condensed level of pain each time Tatsuya heals.
