That's pretty much it,before you start thinking about what you're going to wish for you have to want to make a wish in the first place,and Madoka doesn't want to.
And at the start of the episode Sayaka said she was about to learn the heavy price to pay for miracles,once they know that,wishing Mami back to life just won't be worth the price

I know we havn't seen many Mahou Shoujos in the show,but considering we havn't seen an adult one it makes me doubt any of them live past their teenage years.
