There are clues. The fact that Madoka opens an EXIT door (marked as such) in order to get there, and the fact that the place is not a enclosed space, unlike all the mazes we have seen till now. They're fighting
outside. It's the real world.
The fact that there's at least a known building (Kamijo's hospital) is also a hint of this being the real world.
