Here is the best scan I can provide of the cover. It is not a DVD, rather it is a special binder that has slots for all of the original 8 DVDs that came out in Japan in 2006-2007 and their postcards. It was specially made for a store called "Toranora" and the auction that I bought had gotten his/her set from there and included it in the auction with all the postcards I showed. I too also share your enthusiasm for a Koizumi cover. He is my favorite character afterall.
