Both the manga and the anime made it quite clear that you always pay the toll when you go to the gate. Ed went to the gate three times not twice, first when he tried to transmute his mother, then when he bound Al's soul to the armor then when he escaped from gluttony. The first toll was his leg, the second toll was his arm, and the third toll he paid using envy's stone. There was that whole scene where Ed didn't want to use the philosopher's stone and Envy was saying that it's just energy not people, and to just do it.
