Both. Ilya was born normally as Iris's daughter, but she was also tampered with. Although, it's never made clear what exactly is the cause of her dying a year or so after the Grail War....

Well, if a master is eliminated prior to the start of the war, then their servant will probably find someone else, or else someone else will summon a servant. The masters aren't fixed until the servant has been summoned.
