if this little girl is indeed running away from the doctor, then all those chains would mean that she was captured and locked up somewhere and has escaped somehow...
those two test tubes shown in episode 5...the two empty ones, i'm beginning to think that those two tubes belong to the two new loli's introduced in this episode...
two theories can come up from this...
this new girl can be running away from scaglietti because she was considered a "failure" to him and was locked up because of some mysterious reason and now has run away from him with two relics in her hands and happens to cross paths with two of RF6 already...
or she could be a gimmick for scaglietti...as FlareKnight said, it could be a trap to find out more of RF6 and their S+ mage attacks and abilities and relay back all the info to Scaglietti....and the suitcases that she was carrying...one of them fell into a sewage hole right? what if that suitcase didn't contain a relic, but rather an explosive. so when the time is right, Scaglietti could just detonate that thing and infiltration complete, total chaos for TSAB without any of them even knowing it!
