Sorry, it's from vol 5, the school festival. As for your question
1. It's an event arranged by Tatenashi called reverse Cinderella. The one getting the crown on Ichika's head will be his roommate by the power of student council
2. Cecilia should be there, probably just wasn't shown in the picture
3. That's Charl with Ichika. She first play the hero by defending Ichika from the others and plan to ask the clueless Ichika nicely for the crown after saving him.
4. It's an event by the student council. If the other class is doing maid-cafe or a play, the student council is doing this reverse Cinderella event behind Ichika's knowledge. Oh, and for additional info, the one making the best event will get Ichika for their club. Of course this one is also Tatenashi's idea :P
