2: I'm pretty sure your mind would be too busy focusing on the unbearable pain in your chest while undergoing cardiac arrest (AKA a heart attack) to hallucinate. So it was the hallucination that brought up the attack.

3: From what I can tell that's not the case. But I'd wait for someone who knows both Chinese and Japanese fairly well for a proper answer.

4: Who knows? I wouldn't rule out the possibility of more though.
