Maybe Sekai will stab Makoto when she finds out he went back to Kotonoha, then Kotonoha will saw off Sekai's head for killing him. And then when she realizes what she did, she can commit suicide.
I don't really want that to happen though, I want a Setsuna x Sekai ending. ;-;
