Prior to the deaths of Watanagashi, Shion never assumed the identity of Mion aside Keiichi mistaking Shion for Mion when walking her out and bumping later into Mion. Here Shion never said she was Mion, and was just having fun with Keiichi knowing he hasn't figured it out.

Mion, on the other hand, assumed the idenity of Shion while giving him that lunch box because she was too embarassed to give it to him as herself.

Otherwise, the only other switch took place at the fateful night of Watanagashi which thereafter the murder spree took place, and then the final switch when "Shion" was rescued.
