I bet that scene is Kaede's messed-up memories re-inventing the first time she and Rin had sex (which'd explain the blood). As was said earlier, the game's plot occurs in memories -- often messed up memories. So that'd be my guess... (But have you actually been reading/listening to the story, or are you just clicking through it quickly to get the CGs?)

Pay a bit more attention. A minute or so into the memory of Kaede's birthday party, Primula is mentioned as helping Kaede make some of the food for the party. That would initially set the date of that memory as November 10, 2004 (Kaede's birthday is on 11/10. The arrival of Nerine, Sia, and Primula happened in June 15/16 of 2004, so initially, it's safe to assume that it's currently 11/10/2004).
However, according to Rin, this is incorrect. This memory was actually on November 10, 2003, 7 months before the events in SHUFFLE!, so Kaede's mention of Primula is FALSE.

After the H-scene courtesy of Kaede's scattered memories (Kaede thinks that she and Rin first made love to other on THAT night), Rin counters with the truth. They never had sex on the night 11/10/2003. That wouldn't happen until August 11, 2004
