This is a very good tutorial
But because I'm lazy, I use an abridged version. I just use Levels and then erase all the dirt & fill in the blacks by a combination of the Brush Tool, the Polygonal Lasso Tool and the Quick Mask Tool (press Q after you isolated an area with the Lasso tool).
