Urahara on the other hand was already a SS captain and left; he resides in the living world using a gigai. I don't think it was ever stated that Isshin was ever originally from SS. There's a difference between being a shinigami, and a living person with shinigami powers.

he might be have been using a gigai all that time on earth (like urahara) for all we know
