I believe Kyuichi = Kyon + Yuiichi. It's most likely a joke aimed at the fact that Sugita Tomokazu is yet again voicing the lead character in a KyoAni production.
"Chin of death" refers to the original 2002 Toei production, in which Yuuichi was animated quite poorly, and had this...really pointy chin (which seems to lead to Jay Leno references and jokes).
