Adults usually has twice the dosage of kids (kids in this sense is anyone 12 and below), but not necessarily like that depending on what's in the prescription, and yes, some medicines is given based on body mass, not age.

While vitamins only follow the age rule (and in case of deficiency, the actual requirement). And yes, you can use some handwave that the treatment is specially prescribed according to his need, and nobody (even trained medic) will ask unless they're very, very curious about 'unnecessary detail'.
