I don't think the manga and anime are too different from each other.

Yes, Shion was disguising herself as Mion after she locked up the actual Mion in the underground prison. She switched back to herself ONLY when she talked to Keiichi over the phone so she could move her plans forward. Keiichi however came to the conclusion that he was talking to a psychotic Mion the entire time due to the fact he never saw Shion personally after the ritual shrine incident, but in reality it was Shion he was talking to and meeting the entire time. When Shion was attempting to tourture Keiichi he thought the locked up Mion was really Shion.

Since they're identical twins Keiichi never would have seen past that until the end after Ooishi explained that forensic analysis proved that the corpse found at the well was Mion when it was originally assumed to be Shion.
