I wouldn't say its because he is clueless, just that he doesn't know them well enough to be using their first names. If my understanding is right, its kinda a big deal in Japan.

Plus, if you take into account his memory loss, he has known most of them what? two months? Less in most cases. Who knows, he may now be calling someone he used to call be their first name by their last name because he doesn't know that he didn't before.

Like he know calls her Misaka because he has forgotten that he used to call her Biri-Biri.

It's the more formal way of adressing someone, but I don't think it indicates his cluelessness at all.
