Your right about this one but it happens in vol 16 and in the case for Misaki. I can't remember her name being mention in index series but her name was mentioned in new testament. In vol 21? or 22? I think she was still mentioned as mental out in the afterword of the author.
Her name was only revealed in the manga and then in new testament.
BUT!!! in railgun it should be already mentioned that there are 2 level 5 in tokiwadai
