It's possible that Naruto is related to Tsunade. It was once stated that the Hokage were only those of the Senju clan and Tsunade is Senju, so Minato could have been a branch member...
I always suspected that Naruto was descended from the second Hokage, Tobirama Senju, because they sort of look similar...
Another idea although still in speculation, is that Naruto is a descendant of the Sage of Six Paths....
