And now we get down to the nitty-gritty. Another great episode -- but of course you knew that. Every time I see/hear the OP and ED I am struck by how good they are.
Spoiler for ep5 partial summary:
Rika's condition is weak. She is forbidden visitors, but Tanizaki sneaks Yuuichi in for one minute. Rika is very happy to see Yuuichi. Back at his office, Natsume remembers years before when his own young wife died of a heart condition. Yuuichi goes out at night. He is picked up by a woman I think may be Tanizaki's sister (EDIT: a childhood friend of Akiko's). She has been in Tokyo modelling, but has come back to Ise because her father is ill. She hates Ise. She says she has to go home for something and gets him into her bed, where he is lying dazed with uncomfortable desire. As she starts to unbutton his pants, Tanizaki and Natsume show up. Tanizaki hits him. Then she hits the woman. On the way back to the hospital, Natsume remembers a time when he was in bed with another woman while his own wife was dying, so he doesn't react to Yuuichi's transgression. Yuuichi sees Rika before her surgery and she makes sure he reads the book she gave him, Les Thibault by Roger Martin du Gard. He's sitting on the roof reading it and reads a passage that is a letter from a woman to a man, where the woman says she will gamble her life by becoming his and signs it with her initial. Rika has changed the initial to "R". Yuuichi breaks down crying with shame and grief. Rika is undergoing her operation. Tanizaki tells Yuuichi that Rika has taped a photo of him to her leg for good luck during the operation. When Natsume comes out, Yuuichi asks him how Rika is. Natsume says that her condition is fragile and makes it clear that she is not out of danger. He says she will probably die.
