1. All enemies respawn. But if I had to guess at which ones don't, I'd say the end-boss in dungeons holding rare treasure, and possibly the Four Gods.
2. DMG and the main storyline are connected by that Glass Monarch, an ex-NN member, is the Parent of Chigira Chiaki. Otherwise, neither work shows much else happening in parallel.
3. I don't know.
