Having re-experienced the prologue+part of ch1 in the anime, it got me thinking back to the game and how some things always bugged me about

the final chapter and future Okabe's "Operation Skuld." So was this final grand scheme really free of plot holes?
I guess what really bugged me was:
According to the game, Okabe only "observed" that Kurisu had been covered in blood, he never checked if she was actually dead. Therefore, Kurisu's death is not set in stone and "deceiving" Okabe and the world can save her.
But this is only true from Okabe's perspective... I mean someone else could have "observed" Kurisu's death after Okabe left the building! Was the game trying to say that Okabe's POV is the almighty decider in how the world lines eventually "converge?"
I mean, how can future Okabe be so sure that Kurisu's death is not the fate already decided by convergence of the world lines (much like how hopeless Mayuri was in world line alpha)? And that what Okabe saw was the only thing that mattered?
I'd love you guys' input on this.
