We're going to end up in a circular phase (and fundamentally all of this is conjecture anyway) but you're referencing certain events and instilling a finite barrier (to a certain extent applying real world science) in a reality where science and magic exist.
Misaki 'nuked' the doors to the concept of Misaki within Touma's brain. It doesn't mean that the concept doesn't exist (and can't be developed and added to) it just means he can't get inside because there's no entrance way. It's fundamentally no different to the 'windowless building' requiring a teleporter. Even with Dragon's Breath doing a second round of nuking it doesn't change the status quo about the Misaki concept. The doors still don't fundamentally exist but it doesn't mean the concept cannot once again be built up, accumulating over their various interactions. It just means the concept of a flat chested girl in a swimsuit has been erased from his internal records (that he couldn't access anyway), but now with recent events there's a non-flat chested girl in a swimsuit in his records (that he still can't access). Obviously he's in for quite a treat should he ever find a way in. If a teenager hides porn in their room but you never go into their room and they never tell you about it is that the same as them not having porn in their room? Mental Out exists as a concept, in the same way that all the level 5's exist as concepts independent of their actual 'this is the real person' identities. It's no different to the 6th level 5 that we're yet to met but know exists. Mental Out is essentially an avatar in this sense in the same way that 'Railgun' would be if he didn't doing crazy things and establish a relationship with Misaka (and her subsequent reinsertion into his life post breath). Hypothetical: Misaki is some sort of sadist who beats the crap out of Touma, day after day (maybe stealing his lunch money). Would he have absolutely no recollection that someone beat the crap out of him, or would he know he got the crap beaten out of him just not know who did it? ○ If he has no recollection of the events at all then it'd be the same as waking up day after day being injured with no apparent reason/cause (and that in itself should be scary as hell).

If the writer not only brings it up in their works multiple times, wether by one character or more, but also ties it into a characters development (see Mikoto's' development, mostly because she is the one tied into this theme the most), then it has some relevance. Sure, it may not be the main focus, but the fact that Kamachi even brings it up at all is a sign that Kamachi considers the topic relevant to the story in some way. Now, wether you don't think it matter either due to personal bias (let's not beat around the bush and just admit that this is a factor for some to consider it irrelevant) or because you truly do think it is irrelevant then that is on you and doesn't change the fact Kamachi added it to the story and that it is a present theme that gets mentioned.

In a way it's endearing that Othinus and Touma aren't soul-bound (given how much he means to her), though one has to wonder if traumatic scenes remain as such if the initiating devastation can be undone as if an illusion.
Did the invisible thing in Touma's arm never regenerate after he crushed it against Fiamma? I mean he's missing his right forearm so I would have expected something, or is there some other triggering condition?
