Does anyone know what chapter/volume explain how Mana lost her memory? In wiki of her character it talk about how she lost her memory but I can't seem to find it.

It was during the first meeting with Shidou she states

"I——Truthfully speaking do not have any memories from the past."

"......What is this?"

Regarding this, Kotori revealed an expression of utter displeasure. Adjusting her body posture to face Mana, speaking once more.

"If it is the past that you have mentioned, how much exactly did you lose?"

"That's right. I can't remember anything two to three years prior, but I can still recall incidents past that period."

After Kotori's words, Mana took out the silver locket in front of her chest, within it was an extremely discolored photograph. In that photograph, contained the figures of Shidou and Mana when they were still children.

"Is this......me?"

The reason why is not completely clear, but you can assume Phantom has erased her memory to disguise her truth

I was under the impression that her lost memories were because of the operation DEM performed on her that also shortened her lifespan.

It actually notes that the picture Mana has is much more recent than it should have been. Taken long after Shidou was separated from their mother and adopted by the Itsuka household which should've been the time when Shidou and Mana separated since he was at least 10 years old the picture. Rather than separated I find there is something fishy about Mana. Kotori didn't know her at that time so why was she there? My guess is that Mana could be a form that Phantom takes on. Maybe Phantom intentionally erased her memories to do so. But because she erased her existence from Shidou during the fire, Mana's existence was also erased. Phantom then had "Mana" infiltrate DEM.
