I do believe that Issa is her father, I think Issa just mates with all the strongest vampires from around the world without the connotation of marriage. Wanting powerful off-spring is probably all he wanted to fulfill his ambitions. What kinda begs to question, why haven't he had any sons? I kinda feel bad for him, not being able to have a boy daughters are nice and all, but as a man I would like to have at least one son to bond with

'-san' is also just a term of respect. It would seem weirder than hell if the most powerful vampire couple called each other 'Akasha-chan' and 'Issa-chan'. Akasha seems like a very formal and traditional kind of girl so her use of 'san' shouldn't be too much of a shock.

Um, I'm not sure if this is rhetorical or not
But, if you're serious... well they pretty much made the anime a giant ecchi omake, It took tidbits from the manga, but instead of making of following the plot, they made a standard Ecchi Comedy.
