For anyone else that has read this, can they explain what's going on?

In chapter 11, after getting shouted at by Kokoro, it looks as if Makoto decided to break things up with Sekai since he didn't want to hurt Kotonoha. On the roof at school he seems to tell Kotonoha the truth and begs for forgiveness which she accepts. But then for some reason she decides to try and kill Sekai anyway with a saw. Makoto gets in the way and winds up getting slashed.

Chapter 12 starts off with Makoto in a hospital, surrounded by his friends as he recovers. Sekai soon arrives and stays in his room as the others leave. As Sekai is cleaning up the room, Makoto receives a call on his cell. I'm assuming that it's Kokoro since Makoto looks stunned (most likely he'd being told that Kotonoha's dead). Sekai, who was turned away from him, starts to giggle after he asks her something. This is when he notices a (bloody?) knife in Sekai's bag. She then turns around and definitely looks like she's lost it, possibly because of the near-death experience. As she's telling Makoto something she walks towards his bed while (I'm assuming) Makoto's trying to send some sort of text message before he loses his grip and drops his cell. Sekai then looks like she's about to wrap her hands around Makoto's neck; she's either telling him that no one can get in their way now or she's trying to choke him.

All I know at this point is that Makoto tries to make things right and both girls still go insane

Though it's also possible that Makoto was getting paranoid at that point.

I just don't understand why it has to end it total mayhem ,doing this makes major flaws in their script/story.
Just like how can Sekai is worried about Makoto and goes to visit him in hospital then one second latter has a knife and kills him.

The whole manga ended up a mess. It started out good (or average... at least normal), but decided to go

absurdly bad, for some absurd reason...

It's like the manga-ka drew it just to make a bad end where Makoto suffers... And there's no reason (in the manga) for a bloodbath! Unlike the game/anime...

At least the anime had some buildup that had it pointing it the way it did. Here it just abruptly happened! Also I would have liked to seen what happened to everyone else. I doubt they would just be like "Meh, one girl's dead, one lost her mind, and the guy is either her slave or dead." In the anime the other characters figured that would happen, but here there's no evidence!

I would say that the anime had no buildup too, granted, there was a bit of hatred and stuff but nothing that told me that it could build into a blood bath, when i started watching i though its like kimikiss or and other harem's which are slow to build up but it felt it took a while doing the other part of the story got bored and finished it in 10 mins.
I surpose its inlike many other titles but to be fair it is like that because nobody would do a stupid anime like that, well maybe "hayate the combat butler"

I havent read the manga yet so I cant compare what happened in the manga to the anime BUT there was "some" buildup in the anime that pushed it towards its ending. For instance, the foreshadowing of the knife in.. I forgot what episode, I believe 10, when Kotonoha was dumped and sister walked in to say hi while she was washing dishs or something. I figured something about death or someone getting stabbed will come through since then. This unlike other romance genre because it also falls under "tragedy". Try reading, Tsuiteru Kanojo, romance tradegy manga.

Im with you, i can't read Japanese and to be fair not many manga's get translated at.
At this point in time there isn't a translated version of this, If you really want to read it get a good translator so you have to go word by word which will take sometime but could be worth it .

after reading the raw, im pretty much sure kotonoha is lynched by sekai ;D, at the last chapter in the hospital makoto got a call from kokoro, and she was like desperate while saying that her neechan (kotonoha) is well bleeding or unconscious (i'm not sure about this part), and then after the phone makoto saw this machete in sekai's bag , well so yea, the one who got pawned is kotonohara here.. damn and here i thought the manga will end with good ending, like come on half part of the manga is like all good and stuff, makoto well not being like makoto, but oh well

It was good to come back to this with the manga after having had watched the anime.

The manga better continue. I hate knowing it has an ending similar to the anime. I prefer "good" endings. Besides, Makoto is a lot different here than in the anime. He does not deserve to die (albeit killing him in the anime is a bit too extreme).

Well, there's not really any room for ambiguity, I'm afraid. The last chapter is called "Last Page: Futari no Sekai ~World's End~" and it concludes with an all-black page with the nice word "END" (in English). So yeah... Chapter 12 is absolutely the end.
