Ha. I'm going to sound like a loser for saying this, but my fave is Monster's. My favorite theme with animation is by far FullMetal Alchemist's Rewrite (last season's theme)--I love the waterfall scene.

I've always found this question to be one of the hardest to answer. Sadly Openings and Endings are becoming increasingly good, and abundant. Much to our dismay, our favorites tend to last for a month or two if we're lucky before something pops up and steals our hearts away. Honestly the hardest part is deciding between genres. It's hard not to say Banner of the Stars, from an orchestral standpoint. Jazz would be Code Geass R1... You get the confusion here. I don't know what to choose.
I'll go with what ones impressed me the most and kept me enjoying it.
Opening: Code Geass R1 Opening #1 "Colors". (It was difficult not to say Nabari no Ou... Shalala! <_<)
Ending: I have to go with Kamisama Kazoku's ending, because of all the cutesy and dancy endings... It has the funnier lyrics. If you watch it, you'll get what I mean.
Note: I started making this post... What? 11:30pm maybe? Ugh...

I'm not sure if someone's already mentioned this but I recently I've really enjoyed

"LADY LOVE" by RIZE [Shion no Ou OP] The rock-esque song is a bit surprizing to have on that series but I have to say I really liked it.
"Futatsu no Kodou to Akai Tsumi" by ON/OFF [Vampire Knight OP] is catchy and creepy in a wonderous way.
Oh! Who can forget Nodame's OP-
"Allegro Cantabile" by SUEMITSU & THE SUEMITH [Nodame] yum yum~

My favourite opening/closing song would have to be Namikaze Satellite from Naruto.
Can't remember whether it was an opening or a closing but it was a great song. Nakushita Kotoba was a nice one too.

"Hit in America" from Beck is pretty sweet. "Sakura-Saku" from Love Hina's OP is freaking awesome!

There's also "Tank" from Cowboy Bebop, and then I would be obligated to mention "Battlecry" from Samurai Champloo.

As far as ED songs go, one of my faves is from Naruto "Nakushita Kotaba". I guess I'll mention "Hare Hare Yukai" from HaruSuzu, but who doesn't love it?
