Those games aren't Railgun/Index. Even if they wanted to do that would have announced such an important feature that many fans would care about. That they haven't pretty much seals the deal that they aren't going to allow such a thing even if they wanted to because it would be very unprofitable to sink all those other ships just to appease Mikoto and Index fans in a non-canon game.

That's the ting. Romance is important there, Romance isn't as important in Index/Railgun even though Touma has a very large Harem. What's important is keeping all the ships going, at least until the series end, because of the profits that will be had. Touma and Mikoto merchandise already sells well enough. Revealing they're together wouldn't really effect sales there but it would hurt the sales of all the other girls greatly and would cause a storm of angry fans. Not the most wise things to do when the series still has many years left in its lifespan.

2 things I noticed:

1. Bottom Left screenshot: Yep, that's a Hospital Room. We're playing from Kamijous perspective.

2. Top right screenshot: it looks like Kamijou is conversing with Mikoto and Sate....Sa..SA..SATEN!?

Prepare for singularity, Kamijou and Saten meet in this game....Maybe.

A cameo does not guarantee that they will meet.
(And where did you get that Touma is talking to them in that image?, Mikoto is talking with Saten and she didn't even mentioned him)

Logic would denote that they would actually be looking at eachother if they're talking, but maybe I'm not all that familiar with Japanese games.

usually in visual novels the sprites only look forward to the player, so when characters talk to themselves, the sprites remain the same.

Yeah, that's typical VN style for them to always be positioned that way when talking.

Science (Hidden)? Guess only Accelerator would fit that since Mikoto is clearly Science (Surface) and Kuroko doesn't fit in with Science (Hidden). Still strange that if it is Accelerator that he isn't on the cover since he's very popular. I'm guessing that makes Touma the Magic (Surface) story with Index in a supporting role like Kuroko being in a supporting role for Mikoto's story.

She was always bigger than the other girls, the ones she hangs out with, though. At least height wise.

It is titled "Shirai Kuroko's Secret Theatre" - it was already mentioned on the poster but no one translated it
(funny that they used Saten/Uiharu taking a bath as promotional poster)
