Sailor Moon actually has quite a boring first outfit if you think about it. It's basically a leotard with a skirt attached plus jewelry.
And of course Eternal Sailor Moon's costume is really quite gaudy.

As far as cool battle costumes are concerned I personally like Rayne's from Neo Angelique. Although it does jar with all the other Victorian style costumes. But not as much as you'd expect.

Zero's outfit is so amazing. One of the coolest outfits that ive seen.

I also like Ichigo's bankai outfit. Black never looked so amazing like that.

I think the coolest costume I've ever seen in an anime has got to be Lelouch's Zero uniform in Code Geass. OMG! He looks so awsome in it. Especially without the helmet. My favorite image of Lelouch as his Zero persona is when he wears the full costume, (minus the helmet so I can see his gorgeous face) and he's in his theatric poses with his cool hand movements. It's so captivating and he is so omg amazing when he is like that. His cape and the high curving collar make him look mysterious and villain-like. And the black gloves make his graceful hand movements look so cool. Zero's overall costume design is the coolest looking to me.

these are some very cool costumes from Pandora Hearts, not particularly battle type , but very very cool, i find battle in such kind of clothing is much cooler than battle in a specific uniform or gear

I'd go further and just say almost any action based anime produced by Sunrise in just about any era. If you want military inspired uniforms there's the like of Gundam and Code Geass, if you want outlandish fashion there's Five Star Stories and well Code Geass, if you want fanservicey ones there's Accel World and again Code Geass, if you want something that just looks gar there's Gaogaigar and Tiger and Bunny, and these are just very select examples.

They've certainly helped the cosplay market along throughout the years.

Sayaka Miki has one of the coolest costumes I've ever seen. It combines so many elements that I like.

All of the PMMM main cast have great character designs, and really capture the great appeal of magical girl aesthetics.

Now, for military uniforms, I'm a bit of a retro guy. Give me the Principality of Zeon.
