It's Miyabi. It's not shown in the anime, but THREE YEARS actually pass from the time Kyouma was recruited for Grendel, and the time of Miyabi's operation.
By this point, Miyabi was being held in a special life support chamber to extend her life, while Dr. Yurizaki finished the full-body prosthesis she was making for her. Also, hate the guy, please: Spoiler for Stuff:
Seriously, he makes every effort to be a dick in ALL his appearances from books 6-9.

Thank you for all the info, I'm very excited to see how the anime handles adapting all these chapters. I picked up the English version of volume 1 today, and am still somewhat sad at how much was cut out (mainly for time).
So the series is still continuing right now, and is still on the island arc? Huh, long arc, guess the rest of the season will be about that then. Wonder if it'll show how that arc ends before the manga does, it happens occasionally, I think. Like I think the Seraph of the End anime got further than the manga did, for an episode or two. |

Chapter 75 acts as a sort of epilogue that could be taken as an ending, but it's obvious that the overarching story still has far to go (plenty of untouched loose ends such as the scarred assassin). I think it's quite likely for the anime to rush the island arc to end at the same point. And of course, 2 chapters have already been released at its new home at the other magazine.

Fortunately it looks like both magazines are owned by the same publisher, so Yen's localization license should still be valid for both before and after the switch-over. This has been a problem in the past, with works that changed publishers mid-stream.

This week's episode adapted chapters 42 to 49 by the way. Oh, and they adapted out two other characters too — another Collector who got out with Ellie and Loser (a young one who claims to be a 'fan'), and an old man who went to the cabin with the others. Well, that, and the dead people who didn't get out of Easter Island in time. Brr.
Otherwise, it's almost a straight frame-to-frame adapt.

This week's episode adapted chapters 50 to 55, with minor cuts -- they didn't show any of the scenes with Dr. Yurizaki in it, like this one, where she's actually making Miyabi's new body, but debating whether or not it will really work:'
Otherwise a nearly frame-by-frame adaptation again.
