The fake illness had real symptoms. Araragi transfered over himself half of the symptoms. It all comes down to the fact there is an urban legend in Japan that says that you can cure a cold by infecting another person. Basically you could say Araragi hypnotized himself without knowing.
Apparently Kaiki does not believe in the supernatural, but he is a liar so he can't be trusted. It is certain he knows the knack of occultism. |

Imagine how and what exactly appears to the observer, usually the MC. Imagine their reactions to it. Etc. I don't know if you are capable of doing it (and this is almost a philosophical thing) but you should be able to enjoy it for what it is from the perspective of who is involved.
