It's just Rainmeter... the skin is one I made myself, although the girl is cropped from an image that I got on Pixiv.

If you use WinAmp the buttons interface with it and you can play music.

And it wasn't all that hard. ^^;; Rainmeter is pretty intuitive, you can look up how to make a skin on their site.
