There is a big consequence if they do that, for instance: a massive flop wave for Oshi no ko from amount people who going to ragedrop this manga because they feel betrayed for such heinous plot twist.

Think about it, Oshi no Ko Became big hit because Ai an idol who suffered tragic death for her decision to raised a children then later if author stupidly decide to make her children commit incest...is that good things?
No matter you look that horrible from us readers perspective and there is Ai who is real victim for this she had to died earlier abandoned her children and now must face her children commit an incest what happen to her is beyond tragic at that point.

Put incest is Aka signing suicide letter for Oshi no Ko. It definitely must not happen for a reason.

You're jumping the gun. This might change something for Ruby because avenging Goro was her main motivation, but it's up in the air whether this changes anything substantial for Aqua. He told Ruby to stop seeking revenge but he never said he was gonna do the same. We know he can be pretty hypocritical so we can't make any conclusions just yet.

Aqua doesn't need to completely give up on revenge to change. Just going from pushing everyone away and living on nothing but spite to being a proper guardian for Ruby would still be an improvement.

He won't truly get better unless he stops trying to shoulder everything on his own. That was the root of the problem and the real reason he never opened up to Ruby after all.

Rather than being guardian to Ruby (for better of worse, he's just a kid like her; he can't be her guardian), he should just stop trying to be the "hero" that solves all the problems on his own. Though I don't expect that kind of growth from him.

He will either turn her down or tell her to put the issue on hold until he's done with his revenge most likely yeah although incest subtext is already in play here even if he turn her down.

Don't exactly care to argue about who he will end up with and you seem to have a very firm belief about where the story will end from the looks of it so it will be a waste of time for both of us.

He didn't open up to Ruby precisely because he didn't want her to end up living only for revenge. It happened anyway, so he had no choice but to confess now.

Even if Aqua is still after Kamiki, living for Ruby's sake too would still be progress.

Which gives away the real problem here. He always tries to shoulder everything himself because he has a hero complex. It's not something new. It's the very reason he became a doctor. First, you shouldn't be overgeneralizing being motivated to put others before yourself to be a bad thing.
It can turn toxic if it's one-sided and the other side doesn't care for you back, but there plenty of cases where people are more responsible and motivated to act when they have to support a family, a loved one, or even their pet.

Second, thinking in terms of black-and-white and pushing your own ideas about what you think is "healthy" doesn't help anyone recovering from trauma or their own mental issues either.
We already know Goro and Sarina have issues. If Aqua can go from having Kamiki in his head rent-free 100% of the time to thinking about supporting Ruby even 25% of the time, that's a positive improvement regardless whether that fits your idea of "healthy" or not.

Not regularly keeping tabs on the manga, but I had to look this up because of someone mentioning it. It really took 122 chapters to reveal this thing I've been complaining about since ep 1 of the anime.

The leakers are typically pretty reliable and from what I can tell they're holding back to keep the fandom from melting down.
