So I suppose the witch is really his mom?

That, or Watanuki's parents aren't really dead. Also, maybe Watanuki and Shaoran are splits of the same person? Since something was taken from Shaoran, maybe Watanuki's it?

Or maybe Watanuki's the son of Eriol and whatsherface? Or Tohya and a genderbent Yukito?

Strictly speaking, they are not the same person. In Tsubasa, it's explained that Watanuki is a "replacement" for Syaoran, because his choice to "save" Sakura causes such a great disturbance. I don't think it's stated that Kinomoto and Li are dead.

It may not be an mistake. Heaven knows what the kid had been through, not to mention how his memories had been shaped. I'll be really surprised if Watanuki has no adverse reaction after knowing who he truly is.
