Actually more like a car safety fail. Lorry tires should never break away from the axle like this. Given the weight of the tires, this accident could have been fatal.

Unless the other person is replacing the wheel on a slope.

But you are right, the victim could have had both his shins smashed. A lorry wheel has enough mass to cause broken bones.
