They're doing two things.
1) She's drinking the stuff his mom drinks to keep her boobs wonderous.
2) He's doing the old japanese myth of massaging her breasts to help them grow. Of course, he's wearing kitchen mitts and being completely embarrassed about the whole thing. And she's like, "I can't let anyone else do this... but eek how embarrassing."
