This is during the Mikoto experiment which Kouka and the Antibody Network attacks and Mikoto's hIE body is first destroyed. And yes, Mikoto survives this round and Kouka only finishes the job much later

That's gross oversimplification of things. But at the base level you could say the main antagonist (=/= villian per se) is Higgins and Ryou. The whole thing is a conflict over human-AI relations

Because your usual novel to anime adaptation is extremely condensed?

As an example, Uchouten Kazoku is also a single season in one book. And BEATLESS is actually two books in length, just that the first edition put everything in one volume. Also, the latter part of the story (beginning after Erika gathers everyone), is pretty long despite having one less chapter

Everything Arato has gone through was orchestrated by Lacia so that ultimately she can essentially hack human civilization and put in under automatic control like how Mikoto is the automatization of politics. Deliberately. Everything was literally manipulated by her, meaning not just Arato but those around him and the circumstances they were in, everything that led up to when she was about to implement the mass hack of human society....they were all calculated and planned out by her well in advance.

Yes. That was the most important point actually. At that moment she decided to act on the basis that humans could be trusted and set her aim as making a world where humans and AI can have mutual trust. It's also seen when Arato still did not turn away from her and kept his responsibility as her owner even after she is revealed to be a super-AI and a potential threat to humanity
