Ok, I am glad that they put that last minute ending in the show (phew). Here is my theory (with some ranting) on the matter:

Personally, I am a bit surprised that no one considered that Yasuna NEEDS the relationship as a sort of therapy for treating her illness. However, this does not mean that the relationship is permanent! Yasuna most likely knows that Hazumu either loves Tomari more or considers the way that she got Hazumu was unfair.
There is an extremely high chance that Hazumu only choose Yasuna because she needed her (guilt). If it got to a point where Yasuna didn't mentally need her(him) anymore, would the relationship be the same?
Personally, I think that having faith in the Hazumu x Yasuna relationship is a little misguided: For all we know, hi-hi's theory may actually be correct for what Yasuna said at the end (though I doubt it would be quite that... accurate). For all we know, Yasuna only loved Hazumu because he was the only guy she could see at the time (due to his femininity fooling her 'guy' radar? lol)
Most likely she said something about being able to support herself now and that Hazumu should choose who she(he) really wanted to be with; that he should go to her, or something like that.
In my opinion, I think Hazumu should be with Tomari because Weak Minded Girl (Hazumu) + WeakER Minded Girl (Yasuma) = Error in relationship computation logic... Missing relationship compliment.
Here are the two scenarios in my mind:
1. Hazumu becomes strong willed (Hazumu can support herself) = Yasuma relationship
2. Hazumu cures Yasuma's illness (Yasuna can support herself) = Tomari relationship
Most likely #2 would happen, but you never know. Let's face it, unless Hazumu becomes strong minded, After Hazumu and Yasuma got married, Tomari would probably be living in their house in order to protect them. LOL, that would be funny to see though!
Personally, unless Yasuma gets MUCH stronger, I do NOT want to see her with Hazumu. Just think, if all it took was a tiny slap <cough>among other things</cough> to get her to go off the deep end, what would happen if she ever got in a fight with Hazumu or something. Brings back vivid memories of seeing relationships of the "Oh, you disagree with me! You must hate me! Nobody loves me anymore, Sob, Sob, Sob... WAAAHH!!" type. Of course this is 10x worse because psychological issues would be present from something like that happening, which may even make things more sensitive to trigger. Yasuna went into the depths of despair way too fast and easily in my opinion (I am all alone). WTH it looks like you are running away! of course you are alone, baka baka baka!
No matter how much I look at it, I see Hazumu/Yasuna = Bride/Bride, and Hazumu/Tomari = Bride/Groom. There is only one possible inevitable happy pairing out of the two that can be forseen, even if it is never shown in a anime, manga or something.
Anyway, just finished watching the show and had a bunch of thoughts.

Well, The last episode means that the anime is too fast and they have to slow it down it to make it last. To make this intresting, they might run to some wierd things like why Tomarai has a male's attitude. I think to make this manga last to book 15 or so they have to make twists except Hazumu becoming a boy again(not the point of the story.)
just can't seem to get tomari's "don't throw me away" line out of my head for some reason.
Same here. Even after seeing it a while back there's still an impression in my mind. Just remembering that whole scene almost makes tear up again. This and the firefly scene were the most memorable for me.
